Service Description
The Department of Transportation (DOT) 5-panel urine drug screen is a standard drug test required for employees in safety-sensitive positions within transportation industries regulated by the DOT. This test is mandated to detect the presence of five commonly abused substances: Marijuana (THC) Cocaine (COC) Opiates (such as Heroin, morphine, and codeine) Amphetamines (AMP) Phencyclidine (PCP)
Cancellation Policy
Terms Payment is due at the time of invoice. Payment confirms the appointment. A valid credit card is required to be saved on file. Cancellation Policy: There are no refunds for services. Appointments can be rescheduled up to 24 hours before start time. No call or no-show appointment will not result in a refund. To reschedule your no call or no-show customer will have to book a new appointment for the full price.
